Log in to your electronic health record (EHR) system.

Generate a CAHPS report for the sample month, and save the CAHPS report file for uploading.
If your organization does not use an EHR system, you can create a CAHPS patient information file manually. The in your 91福利社CAHPS portal provides a downloadable File Specifications Template and guidance document for creating patient information files.

Select the Admin Tools tab at the top of the screen, and select Upload File from the drop-down menu.

On the Upload File page, select your location and click Choose File to select your patient file.

Click the red Upload button to complete the upload process.
