When completing an OASIS assessment, document the patient’s need for process measures by selecting the new Need for Process Measures problem statement under Orders for Discipline and Treatment.

Once the problem statement is selected, identify the corresponding interventions and goals based on the patient’s needs. To document interventions using a custom template, select Additional Process Measures.

Select the appropriate goals to address the identified process measures, mitigate risks and optimize patient outcomes.
